Claims paid within 48hrs

How to Claim: Required Documents

Deceased Documents

  • Three (3) certified copies of National ID card
  • Three (3) certified copies of death certificate
  • Current payslip
  • Doctors’ death confirmation letter (BMD1)
  • Body confirmation letter from mortuary

Beneficiary Documents

  • One (1) certified copy of national ID card
  • Bank statement
NB: All Documents must be submitted to the office within 6 months of the deceased date.
